Sports physicals | $25

To receive a physical:

• Bring completed IHSAA form (available in High School Office, Athletic Office, & Elementary Office)

• Parent/legal guardian, complete consent to treat form online (see link below) or at event; parent/legal guardian not required to be present if child brings signed consent form

• $25 will be due at time of service for all physicals. Cash, credit card, and HSA / FSA cards accepted. Insurance will not be billed for physicals at the event.

IMMUNIZATIONS: If your child is due for immunizations, Open Door or the school nurse will contact you about the opportunity for vaccination at this event.

Children’s immunizations are no-cost for those covered by Medicaid/HIP/Hoosier Healthwise, as well as those without insurance. Private insurance will be billed when applicable, and the insured may receive a bill.
