Sports Physicals

• Parent/legal guardian must be present

• Bring completed IHSAA form (copies available at event)

• No cost

Immunizations

• Children’s immunizations are NO-COST for those covered by Medicaid/HIP/Hoosier Healthwise, as well as those without insurance.

• Private insurance will be billed when applicable, and the insured may receive a bill.

Physicals and immunizations provided by Open Door Health Services. Open Door staff will be onsite to schedule follow-up appointments and/or well-child visits, if needed. Free insurance enrollment assistance will be available for families and individuals, provided by Open Door's Indiana Navigators.
